We provide an Individual Plan Coordinator for each of our consumers. This person is the primary contact who is assigned to work
directly with the consumer and their IP Team.
The Individual Plan Coordinator:
Is responsible for coordinating the implementation of
the 30- Day Initial and yearly Individual Plan for each consumer they serve
Ensures consumer
satisfaction by checking with the consumer and the team to assure satisfaction with our services.
May
ask the team to meet during the year if needed or requested.
Uses IP Teams to help provide services for their consumers such as Placement,
Individual Planning, Safety and Crisis Intervention.
When a consumer's life situation requires involvement in one
of the IP Team's areas of expertise, the Individual Plan Coordinator will coordinate a meeting.
