Community Supports include any individually-designed service, or assessment of the need for service, that will assist a consumer to
live more independently in the community of his/her choice within the dollar cap established.
Program Eligibility:
Must be determined
eligible for developmental disabilities services.
An adult (18 or over).
May not be a recipient of Waiver Funds if residing in a nursing
home.
May not receive any other DDP-funded service except those services related to fulfilling equipment needs.
Types of
Training:
Depending upon the needs of the individual, any one or more than one of the following services may be included as a Community
Support service: Homemaker, Personal Care, Respite, Residential Habilitation, Day Habilitation, Prevocational Habilitation,
Supported Employment and Environmental Accessibility Adaptations. Other services may include Transportation, Specialized Medical/Adaptive
Equipment and Supplies, Adult Companion Services, Private Duty Nursing, Social/Leisure/Recreational Supports and Health Maintenance.
